The settlement rises among the hills of central Irpinia, in the middle Calore valley. Located in a highly seismic district, Paternopoli was hit by the earthquake of 23 November 1980 which caused several victims, injured and many displaced people.

History
The toponym derives from "Paternum", name given in medieval times to the hill which became a refuge for hermits called "Patres" who became an important reference for the poor people of the area. It was called "Paterno" until 1863.
